A simple evidence plan
Connect each important decision to a measure, source, owner and review point.
Collect evidence because it will change a decision—not because a dashboard could display it.
Use one row for each decision
| Decision | Evidence | Source | Owner | Review |
|---|
| What might we change? | What would help us decide? | Where will it come from? | Who checks quality and meaning? | When will we act? |
Example
| Decision | Evidence | Source | Owner | Review |
|---|
| Simplify the onboarding form? | Completion, time, abandonment reason and support contacts | Service events, short user interviews and support log | Product owner | Two weeks after release |
Keep the plan disciplined
- Start with the few decisions that matter most.
- Combine quantitative patterns with qualitative context.
- Record important gaps and limitations.
- Agree thresholds before results create pressure to reinterpret success.
- Stop collecting information that nobody reviews or uses.
